ABSTRACT

Objective:To acquire the impact of COVID-19 pandemic on the operation of tertiary maternal and child health(MCH) hospitals in China, for decision making support of health administrative departments and hospital managers.Methods:The National Maternal and Child Health Institutions Resources and Operations Survey Direct Reporting System was used to collect the resource allocation, workload, treatment quality, work efficiency and asset operation of the tertiary MCH hospitals in China in 2019(pre pandemic) and 2020(during pandemic). Statistical descriptions were made using median.Results:In 2019 and 2020, the number of tertiary MCH hospitals in China was 236 and 258, respectively, and their relevant data were analyzed. In terms of resource allocation, the number of health technicians in 2019 and 2020 was 560 and 548, respectively, and the actual number of available beds was 308 and 305 respectively. In terms of workload, the annual outpatient visits in 2020 were 337 990, a decrease of 23.6%from that in 2019; The total number of emergency visits was 28 997, a decrease of 32.5%; The total number of discharged patients was 13 673, a decrease by 20.5%; A total of 4 723 training sessions on MCH were held for primary institutions, an increase of 1.2 percent. A total of 1 953 724 primary-level health technicians were trained, an increase of 175.2 percent. In terms of work efficiency, the average length of hospital stay of discharged patients decreased from 5.56 days in 2019 to 5.00 days in 2020. Bed utilization rate decreased from 88.90% to 69.15%; Bed turnover decreased from 53.69 to 44.22. In terms of treatment quality, the critical illness mortality rate of inpatients was 0.37% in 2020, 0.11% lower than that in 2019. The in-hospital mortality rate for neonatal patients was 0.04%, a 0.03% drop. In terms of asset operation, the total revenue in 2020 was 248.355 million yuan, an increase of 4.46% compared with 2019, in which the proportion of financial subsidies increased from 11.26% to 15.72%.Conclusions:The in-hospital services and institutional health care services of tertiary MCH hospitals in China were downsized by the COVID-19 pandemic, while the work efficiency was relatively stable, along with acceptable resource allocation, good treatment quality and asset operation.

ABSTRACT

Objective:To analyze the status of secondary maternal and child health hospital accreditation in China and the impact of such accreditation on their service improvement.Methods:The paper used the propensity score matching method to match the accreditation group and the non-accreditation group in 1∶1 ratio, and compared the related indicators of service capability of maternal and child health hospitals between the two groups by non-parametric test and Chi-square test.Results:A total of 842 maternal and child health care hospitals were matched. A comparison was made between the accreditation group and the non-accreditation group, and the differences of the following indicators of individual services within the hospital were statistically significant( P<0.01). These indicators were annual emergency attendance, the number of types of health services for women and children, the utilization rate of beds, the average daily hospital bed of each practicing physician; the following indicators of service management within their coverage were statistically significant: namely the proportion of institutions carrying out health education evaluation within their coverage, that of institutions producing uniform " Mother and Child Health Handbook", that of institutions carrying out information quality control, as well as analysis and utilization.These indicators of the accreditation group were higher than those in the non-accreditation group( P<0.01). Conclusions:Given the role played by maternal and child health institution accreditation in promoting secondary maternal and child health hospital regarding better service content, quantity, efficiency and the quality of business management within their coverage, there is still room for improvement.

ABSTRACT

The construction of Health Hospital will become an important opportunity to promote the strategy of Healthy China. The authors analyzed the connotation of Health Hospital, and the feasibility of building Health Hospital from four aspects, including integrating health policy, building healthy environment, implementing health actions and optimizing health services. Then, six essential factors of building Health Hospital were put forward, namely, establishing a green environment, improving the operation mechanism, disseminating health culture, developing information technology, creating intelligent health care and strengthening top-level design.

ABSTRACT

ObjectiveTo investigate the current status of knowledge, attitude and practice of nurses in mental health hospitals on the prevention and control of nosocomial infections, and to analyze the influencing factors. MethodsFrom June to August 2020, 402 clinical nurses from 8 mental health hospitals in a city were enrolled using simple random sampling method, and were assessed using a self-compiled knowledge-attitude-practice questionnaire related to the nosocomial infection prevention and control, thereafter, the influencing factors were screened. ResultsThe scores of knowledge, attitude and practice dimensions in relation to the nosocomial infection prevention and control among the nurses were (8.43±1.84), (76.01±5.70) and (57.42±3.75), respectively. Univariate analysis showed that the knowledge dimension score demonstrated significant difference among nurses of different types of hospital, grades of hospital, professional titles, education levels, and emphasis degrees of hospital and individual on nosocomial infection (P<0.05 or 0.01). The attitude dimension scores yielded significant difference among nurses of different types of hospital, position, grades of hospital, and emphasis degrees of hospital and individual on nosocomial infection (P<0.05 or 0.01). The practice dimension scores manifested significant difference among nurses of different types of hospital, grades of hospital, professional titles, emphasis degrees of hospital and individual on nosocomial infection, and training frequency (P<0.05 or 0.01). Multiple linear regression analysis showed that professional titles and individual emphasis on nosocomial infection entered the regression model of nurses' knowledge dimension of nosocomial infection prevention and control (P<0.05 or 0.01). Individual emphasis on nosocomial infection, hospital types and position entered the regression model of attitude dimension (P<0.05 or 0.01). Individual emphasis on nosocomial infection, hospital types, professional titles, and hospital emphasis on nosocomial infection entered the regression model of practice dimension (P<0.05 or 0.01). ConclusionNurses in mental health hospital have a positive attitude towards nosocomial infection prevention and control, but their knowledge base needs to be strengthened.

ABSTRACT

Objective@#To acquire the main result quality indicators status of tertiary maternal and child health(MCH) hospitals in China, supporting the decision making.@*Methods@#The National Maternal and Child Health Institutions Resources and Operations Survey Direct Reporting System was used to collect the management operation, inpatient death, re-return, hospital-acquired infection, surgical complications and patient safety of these hospitals in China in 2017. Statistical descriptions were made using average, rate and composition comparison data.@*Results@#The proportion of health technicians of tertiary MCH hospitals in China was 83.66%, the average hospitalization days were 5.96 days, with bed occupancy rate of 90.01%. The success rate of neonatal resuscitation was high. Meanwhile, the mortality rate of hospitalized maternal critical illness, the total hospitalization mortality rate of neonatal patients, the incidence of complications in surgical patients, the incidence of neonatal injury and the incidence of birth injury in vaginal delivery were higher in the institutions that had not participated in the MCH hospital accreditation.@*Conclusions@#The management of tertiary MCH hospitals in China was in good condition, and the relevant policies and projects have achieved remarkable results. Compared with general hospitals, hospital infection and re-return indicators were good. The quality and safety of tertiary MCH hospitals which have not participated in the MCH hospital assessment were poor. It is recommended to carry out MCH hospital accreditation to improve quality and safety.

ABSTRACT

Objetivo: identificar a percepção dos trabalhadores de um hospital acerca da cultura de saúde e segurança no trabalho. Método: estudo observacional, descritivo, transversal, quantitativo, realizado em um hospital de médio porte, no interior de Goiás. O instrumento de coleta foi o Inventário de Clima Organizacional de Segurança. A coleta ocorreu no primeiro semestre de 2016. A análise foi feita através do programa SPSS 20.0. Resultados: participaram do estudo 118 colaboradores, com média de vínculo com a instituição 2,93 anos. Predominantemente do gênero feminino e com idade média de 35 anos. Foi identificado média geral de 2,92 horas de capacitação. Com relação as dimensões do inventário, a equipe de enfermagem apresentou um escore satisfatório nos itens clima de segurança, segurança como valor organizacional e envolvimento pessoal com segurança, enquanto que, nessas mesmas dimensões, a equipe de apoio se caracterizou como indiferente. Na dimensão práticas organizacionais a equipe de enfermagem, mostrou-se indiferente enquanto que a equipe de apoio se apresentou insatisfeita Conclusão: evidenciou-se diferença no envolvimento da percepção da cultura de saúde e segurança no trabalho nesse hospital, ficando evidente a necessidade de capacitação adequada para as categorias que compõem o quadro de trabalhadores.


Objetive: to identify the perception of employees of a hospital about the culture of health and safety at work. Method: observational, descriptive, cross-sectional, quantitative study carried out in a medium-size hospital in the interior of Goiás. The instrument of collection was the Organizational Safety Climate Inventory. The collection occurred in the first half of 2016. The analysis was done through the SPSS program 20.0. Results: 118 employees participated in the study, with an average of 2.93 years. Predominantly of the feminine gender and with average age of 35 years. A general average of 2.92 hours of training was identified. Regarding the dimensions of the instrument, the nursing team presented a satisfactory score in the items Safety Climate, Safety as organizational value and Personal Involvement with Safety, while, in those same dimensions, the support team was characterized as indifferent. In the Organizational Practices dimension, the Nursing Team was indifferent while the Support Team was dissatisfied. Conclusion: there was a difference the involvement perception of health and safety culture at work. Therefore, the need for adequate and uniform training for the categories that make up the workforce is evident.

El análisis fue realizado a través del programa SPSS 20.0. Resultados: participaron del estudio 118 colaboradores, con promedio de vínculo con la institución 2,93 años. Predominantemente del género femenino y con edad media de 35 años. Se identificó un promedio general de 2,92 horas de capacitación. Con respecto a las dimensiones del instrumento, el equipo de enfermería presentó una puntuación satisfactoria en los ítems Clima de Seguridad, Seguridad como valor organizacional y Participación Personal con Seguridad, mientras que, en esas mismas dimensiones, el equipo de apoyo se caracterizó como indiferente. En la dimensión Prácticas Organizacionales el Equipo de Enfermería se mostró indiferente mientras que el Equipo de Apoyo se presentó insatisfecho. Conclusión: se evidenció diferencia em la participación de la percepción de la cultura de salud y seguridad en el trabajo en esse hospital, quedando evidente la necesidad de capacitación adecuada para las categorías que componen el cuadro de trabajadores.

ABSTRACT

Estudo objetivou analisar a estrutura e os conteúdos das representações sociais de enfermeiras(os) acerca do cuidado à vítima de violência doméstica. Participaram enfermeiras(os) de dois hospitais públicos de médio porte do município do Rio Grande/RS. Os dados foram colhidos por meio de evocações livres frente ao termo indutor "cuidado à vítima" e analisados por software. As(os) enfermeiras(os) possuem as representações centradas no cuidado psicossocial demonstrado pelos termos presentes no núcleo central. Na zona de contraste infere-se a presença de um subgrupo que reconhece a importância do cuidado físico, sem se limitar a ações tecnicistas. Na periferia evidencia-se a face com o trabalho frente aos termos assistência e orientação. Profissionalismo revela a necessidade de abordar o objeto com base no saber reificado. Considera-se que por se tratar de um tema com carga emocional, somado à hospitalização e a fragilidade da vítima, o enfoque psicossocial é mais significativo no contexto do cuidado.


Our study aimed to analyze the structure and contents of nurses' social representations about the care provided to the victim of domestic violence. Nurses from two medium size public hospitals at Rio Grande/RS participated in the study. We collected data through free evocations of the inductor term "care for the victim" and analyzed them by software. The nurses had representations centered in the psychosocial care demonstrated by the terms present in the central nucleus. We inferred the presence of a subgroup in the contrast zone that acknowledges the importance of the physical care, without limiting to the actions of technicians. In the periphery, we inferred the work is evident when facing terms of assistance and orientation. The professionalism reveals the need to address the object, based on the reified knowledge. Because this is a theme with a considered emotional load, added to the hospitalization and the fragility of the victim, the psychosocial focus is more significant in the care context.

ABSTRACT

In order to advance the public hospital reform, the performance tests and management were strengthened to promote the rapid development of the hospital. The status of average daily bed ocupation, the number of outpatients and the natural birth rate increased while the drug proportion declined. Some departments were the top level in Hebei, departments of gynaecology and pediatrics were in the leading level in Tangshan.
